Are Recurring Machine Failures Really “Random”?

Over time, recurring failures often become accepted as part of normal production life:

  • Drive or automation disturbances

  • Process variations that seem to appear and disappear without a clear reason

The challenge is that recurring problems rarely exist in isolation.

In many cases, production, maintenance, electrical, automation, and process personnel each see only part of the overall picture. The same issue may be discussed within different groups without anyone fully connecting the observations into a common root cause.

Temporary fixes and operator workarounds often keep production running, but the underlying problem remains unresolved.

Over time, seemingly small adjustments become permanent operating practice. Parameters are modified, alarms are disabled, timing values are adjusted, or operating procedures are changed. While these actions may solve the immediate symptom, they can also shift the problem elsewhere in the machine and create new limitations that are accepted as normal.

One common misconception is that recurring problems are caused by software.

In reality, control software rarely changes or fails on its own. When a recurring problem appears, the software is often reacting exactly as designed. The underlying cause is frequently found elsewhere: mechanical wear, sensor issues, instrumentation inaccuracies, drive performance, process variations, or equipment condition.

The important questions are:

  • Do you know the actual root cause behind recurring failures?

  • Are apparently separate problems actually connected?

  • How much production efficiency is being lost through repeated disturbances?

  • Have the problems been investigated systematically enough?

  • Could hidden interactions between process, automation, and machine functions be triggering the issue?
